Assumption of Risk and Release

Assumption of Risk & Release Form If you require an Assumption of Risk & Release Form for your event or activity you must first contact Risk Management for a risk assessment of your event and an evaluation of the requirement for an Assumption of Risk & Release Form. If Risk Management has already conducted its risk assessment and is now requiring an Assumption of Risk & Release Form for your event, please follow the contract review process by submitting a Contract Routing Form form with a request to “draft” and attaching a memo fully describing your event or activity. |
