and send this form and a team participation fee (up to 12 members*)
of $90, if before 10/31/03, and $100 afterward, to:

*Maximum team size is
12 students. If you have more than 12 students, please form another
team (or teams) and include Participation Fees for each.
PLEASE
NOTE: A
fee of $5 per student will be collected on or before the
day of the Challenge (March 6, 2004). This fee covers
