When you arrive at the center for your first visit, you will receive a packet of paperwork that needs to be completed.  The paperwork that you will be given includes:

1. an informed consent statement
2. a demographic questionnaire
3. An inventory of presenting symptoms/concerns
4. a scheduling form

This provides important information to the center about your history, your family, and your current concerns.  Individuals who utilize our services are encouraged to review the informed consent statement as it describes general counseling center policies, what to anticipate by coming to counseling, the limits of confidentiality, and the risks and benefits associated with participating in psychotherapy. Should you have any questions, consult with your counselor at the first appointment.  Some clients find this paperwork burdensome, so thank you in advance for your cooperation.
