Use this checklist to make sure you are submitting a complete MAFP application by the application deadline. The following documents will complete your application. Remember, you will be submitting part of your application online, and providing us supplemental materials to complete your application by mail.

MAFP Application and Supplement Document Checklist
Priority consideration deadline: December 5, 2008
Final application deadline: January 5, 2009
application part one:
- Complete and submit the online application. Click on 'Apply for Admission'. (The University of Denver application code is FPSY, the major is FORENSIC PSYCHOLOGY, there is NO concentration code, the degree code is MA.)
- Pay your $60.00 application fee online via credit card, or include a check or money order in US funds made out to the University of Denver with your supplemental materials.
application part two:
- Respond to the MA application essay questions thoroughly and completely and include them in your packet with your MAFP Supplemental Application.
- Take the GRE General Battery and submit your scores to the University of Denver - School Code: 4842, Clinical Psychology code: 2001. The GRE psychology subject test is optional for MAFP applicants.
- Collect your three (3) letters of recommendation in sealed envelopes with your referee's signature across the flap.
- Collect two (2) copies of ALL your official transcript(s) and either enclose the sealed envelopes in your application packet, or have them sent directly to the address above. NOTE: Do NOT wait for final transcripts with your degree posted if you are still in school. We realize you have courses in progress during the application process. If you are offered admission, you will have to provide final transcripts prior to matriculation.
- Complete all prerequisite classes or submit syllabi with your application indicating completion of coursework. NOTE: A GRE psychology subject test score of 660 or higher will suffice instead of four (4) psychology courses.
