Curricular Practical Training (CPT)

I. Introduction

To apply for Curricular Practical Training please download and fill out the CPT packet below. Please note that the forms can be completed on-line using Adobe Acrobat Reader. To download these forms you MUST have Adobe Acrobat installed on your computer. Click on the Adobe Icon below to download the latest version.

To ensure that your authorization is prepared for you in a timely fashion please ensure that you have all forms to International Students and Scholars Services (ISSS) at least 5 business days before your intended starting date.  Please also remember we must have all forms properly completed and turned in before we can process your request.

II. Application Process 

1. Download CPT Packet: The first step in the application is to download the forms and information from this page. Read them very carefully.
2. Ask Questions: If you have any questions about the application or application process, please contact an ISSS advisor.
3. Submit Materials: Once you have completed the application you can submit your application to the front desk of I-House.
