Membership Forms

New Club Members

New Members are always welcome to join the DU Curling Club! You are considered a new member if you have not paid any Curling Club Dues within the past four years. In order to join, we will need you to fill out a Member Information Form, A Release From Liability Form, and a Verification Form (only if you have taken the DU Defensive Driving and/or have CPR/First Aid Certification). These forms are found below. To join, simply fill out the registration packet and attend a club meeting - at that time you can pay your club dues and you will then be a full-fledged member of DU Curling.
