Report Trouble with a Phone

This form may be used by faculty, staff and students to report trouble with any telephone on either the University Park or Park Hill Campuses

Resident students should follow the trouble-shooting instructions on the Telephone Repair web page before submitting a trouble ticket report.

Required fields are labeled with an asterisk.

Standard Trouble Descriptions (you may choose more than one):
Hear constant ring instead of dial tone	Lights not working (describe below)
No dial tone, dead	Phone doesn't ring
Can not break dial tone	Keypad not working
Can not get outside line	Damaged phone (describe below)
Cuts off in mid-conversation	Noise/static during conversation
Other party can't hear you	Feature not working (describe below)
