2014 - 2015 Wellness Incentive Program

wellness incentive program  (for employees covered by a DU sponsered health plan)

At DU, we believe that an individual's health is their greatest asset. To support you and your family in caring for your greatest asset, we offer a significant premium credit for individuals who complete the Wellness Incentive Program.

NEW EMPLOYEES (COVERED BY A du SPONSERED HEALTH PLAN):

If you (and your covered spouse/partner) complete the program, within 90 days from the start of your Kaiser health plan, you will receive a premium credit ($22.91/month for employee-only and employee+children coverage and $45.83/month for employee+spouse/partner and employee+family coverage) for the plan year beginning July 1, 2014 and ending June 30, 2015.

If your spouse/partner is covered on the DU KP plan, you MUST BOTH complete the THA AND biometric screening to receive the incentive.

To earn the incentive, you (and your covered spouse/partner) will need to complete two components:

  COMPONENT 1:  BIOMETRIC SCREENING

  • Blood pressure
  • Total Cholesterol
  • Blood sugar (glucose)
  • Body Mass Index

You may visit a KP clinic to complete your biometric screenings, which include blood pressure, cholesterol, glucose, and body mass index (BMI). To obtain credit for the DU wellness incentive, you must submit a verification form signed by your physician stating you have completed your screenings at the medical office. Download the verification form in the link below. This verification form does not contain your screening outcomes, only your name and completion date.

The easiest method for completing your biometric screenings through KP is to email your Primary Care Physician, who can direct you to either schedule an appointment with primary care, or can put in a lab order for your blood draw. Let your physician know that you are participating in a wellness program with your employer (DU) and are required to have a lipid profile, fasting blood sugar, blood pressure and BMI screenings completed. Let your physician know that you will need his or her signature on a confirmation form stating that your screenings are complete, and ask as to the best method for getting that form to your physician (by fax, mail or in person at an appointment) for his or her signature.

To qualify for the incentive, you will need to return the signed form to Mandy Sigmund in Human Resources, within 90 days from the start of your Kaiser health plan. Verification Form

 

 COMPONENT 2: TOTAL HEALTH ASSESSMENT (THA)

Sign-in to kp.org to take this 20-30 minute personal health assessment.

Once logged in, find the "Total Health Assessment" link in the 'New members: Get started' section.

We recommend you take the THA after completing your biometrics to receive the most accurate report of your health risks. This component must be completed within 90 days from the start of your Kaiser health plan.

Your results will be confidential; DU will only receive notice that you have completed the THA and biometric screening.

Employees that enrolled in a DU Health PLan during open enrollment (May 1-15,2014)

The wellness manager will be in contact this June with employees that enrolled in a DU Sponsored health plan during open enrollment. You will be able to participate in the program once your Kaiser health plan begins on July 1, 2014.
