Section 1: "From" - Print your name on the Sender's Name line and University of Denver on the Company line. To insure the maximum discount, leave "Sender's FedEx Account Number" field blank.
Section 2: "Your Internal Billing Reference Information." - Write your Fund and Organization number in this field.
Section 3: "To" - Do not enter PO boxes for delivery addresses.
Section 7: "Payment" - Check the Third Party Billing box and write our third party account number in the field. (If you don't know this number, contact Tera at x16685)
