1. Enrollment into the Wellness Counts program closed on April 15, 2013.
2. If you enrolled by April 15th, you will receive a welcome letter in the mail from Kaiser Permanente that will describe the program and provide instructions on how to complete the program. This letter may take up to 2 weeks to arrive.
3. Complete the preventive screenings and Total Health Assessment required specifically for you by May 15, 2013.

Note: If you cover a spouse/partner on your insurance, they needed to enroll separately by April 15th and complete all Wellness Counts program requirements by May 15th to earn the insurance premium incentive for the 2013-2014 plan year.
