2013-2014 Wellness Incentive Program

We're serious about your health and well-being at DU. In order to best understand and meet the needs of our employees, we offer a significant premium credit for individuals who complete the online Total Health Assessment offered by Kaiser Permanente ($250 annually for employee-only and employee+children coverage and $500 annually for employee+spouse/partner and employee+family coverage). You cannot take the THA online until your KP coverage begins.
How to take the online Total Health Assessment (THA):
1. Once you have your Kaiser Health Record Number or ID cards, login to the Total Health Assessment using your user ID and password. If you do not have an ID or password, you will be prompted to create one.
2. Click, "Take the Total Health Assessment now" to get started.
3. DU will automatically receive a list of who has completed the THA. The University only receive aggregate data, identifiable information will NEVER be shared) and will apply the credit to your next paycheck (payroll deadlines permitting).
Note: We cannot back-credit your account. The sooner you (and your covered spouse/partner) complete the THA, the sooner we can get your credit applied. Please expect a 1-2 month delay from when you take the THA to when your credit is applied (example: if your coverage began September 1st and you complete the THA by September 30th, you will see your credit on your November paystub).
Reminder: In order to receive the premium discounts you and your spouse/partner (if covered by a DU sponsored KP plan) must complete the THA before a credit will be applied to your account. Please direct your covered spouse/partner to complete the steps listed above.
CURRENTLY ENROLLED IN A KP PLAN THROUGH DU?
Enrollment into the Wellness Incentive Program (Wellness Counts) is now closed for current employees with KP coverage through the University. Unless you have an IRS qualifying event, you are not eligible for the program until the next time it is offered to all plan members. Please look for program details in the DU Digest in early 2014.
