• Online/Distance Learning ID Request Form

    Please use this form ONLY if you are an online or distance learning student and are currently enrolled. If you submit the form as any other type of student your card will not be mailed, nor will the form and picture be returned to you. Once your paperwork is processed, we will mail your Pioneer Card to the current mailing address on your DU records.

    All users must complete a New User Request form and an Access Request form to receive access to the Pioneer Card system.

      All users (except Sodexo) must begin by completing this form.
    • New Sodexo Pioneer User Request
      All Sodexo users must begin by completing this form.

    Access Request Forms

    • Security Access Request
      Complete this form if you need to view/control access to specific card access doors on campus, including building and room access, or if you need to view location reports.
    • Sodexo Access Request
      This form is only for Sodexo employees. Complete this form if you need to update product, price, menu, tax, or bar code info or view location reports.
