Health Benefits Annual Notices
The annual medical benefits notices include important information about your health coverage and legal protections. These may include:
- HIPAA Privacy Practices – How your health information may be used and your privacy rights
- Women’s Health and Cancer Rights Act (WHCRA) – Coverage for reconstructive surgery following mastectomies
- Newborns’ and Mothers’ Health Protection Act (NMHPA) – Protections related to hospital stays following childbirth
- Summary of Benefits and Coverage (SBC) – Overview of plan coverage, costs, and key features
- Patient Protection Notices – Information regarding provider choice and primary care designation

403(b) Retirement Plan Annual Notices
The annual 403(b) notices provide key information about your retirement plan, including:
- Summary Annual Report (SAR): A narrative summary of the Plan’s financial information for the year.
- Qualified Default Investment Alternative (QDIA) Notice: Information about the Plan’s default investment choice and the circumstances under which your assets may be invested in this investment.
- Notice of Universal Availability and Retirement Plan Contribution Limits: Notice of your right to make elective deferrals into the Plan and the contributions limits applicable for this year.
- Participant Fee Disclosure: Information about the administrative and investment costs of participation in the Plan.

- Summary Plan Description (SPD): A summary of the Plan’s terms and how it operates.
- Summary of Material Modifications (SMM): a description of recent changes to the Plan and changes to information included in the SPD.
