Submit a Request

 

Prior to filling out the form, make sure to; 

  • Consult with your supervisor about your unit’s specific Flexible Work Plan. 

  • Fill out your Home Address, Offsite Work Address or Designated Work Address if you choose the Remote or Hybrid option (the form will not allow you to continue unless the address is saved in your profile). You can do this adding an “off Campus work address” under “My Person Profile” at the MyDU portal

  • Make sure all of your Employee Information is up to date. If not, please contact your unit’s Business Officer before filling out the form.
